I consent the student(s) I have registered or anyone in their household will not attend class or visit the studio if they are showing any of the following COVID-19 symptoms:

Fever Difficulty breathing
-
New onset of cough Difficulty swallowing
-
Unexplained fatigue Pink eye
-
Chills Nausea, vomiting, diarrhea, stomach pain

Headache Sluggishness or loss of appetite
-
Sore throat Lost sense of taste or smell
-
Runny nose Stuffy or congested nose
I consent the student(s) I have registered or anyone in their household will not attend class or visit the studio if they have tested positive with COVID-19.
I consent to follow all guidelines given by public health before returning to class if isolation is required.
As the Parent/Guardian of the student(s) registered I agree & consent to the following Studio Policies:

Payment will be made in accordance to what is agreed upon by Port Dover Dance Academy.

Payment will be made by email transfer to pddastaff@gmail.com, cash or cheque to hold your spot. Spots will NOT be held in class until payment is made according to payment options agreed upon by Port Dover Dance Academy.

In the event of a Provincial Lockdown classes will be moved online to Zoom.

Refunds or credits will not be issued if you choose to not participate in the online Zoom classes if we are forced into that by federal and/or provincial and/or local health unit authorities.

If you register for a class, hold a spot with payment & then you decide for your child or you to not attend classes, whether it be before classes have begun or once classes have begun, NO FORM of refund or credit will be issued because you changed your mind.

You agree to pay a costume fee of $125 per each class that you or your child is registered for by October 15/22. Costumes will be ordered in November 2022 to ensure delivery on time. Room for students body growth will be considered. Your costume(s) will not be included in the order if the costume fee for each class has not be paid in full. All costume sales are final with no option for refund, exchange or credit.

If your child is old enough please send them into the studio alone to limit the number of bodies in the studio while our waiting room is closed. If your child is 2-5 years of age and requires 1 parent to enter for drop off please do so as quickly as possible and then return to wait outside until class is over.

At this time, we are operating classes at full capacity without social distancing. This policy may change throughout the season according to provincial and/or federal government and/or public health authorities.

You agree to follow the studio masking policy for students 2 years & older. This policy may change throughout the season according to federal and/or provincial government and/or public health authorities & you agree to adhere to the policy as it changes. If you do not agree with the masking policy changes & choose to withdraw your child from classes NO FORM of refund or credit will be issued because you changed your mind.

At this time, masking in the waiting room & during class is a choice made by each family individually. PDDA will support each student & their family regardless of their masking choice.

Registered students 12 years of age and older, parent participants, adult students 18 years of age and older, volunteers, assistants & instructors may be required to provide proof of full vaccination plus 14 days to be shown through enhanced vaccine certificate with the QR code to enter the studio & participate in studio classes/activities if the federal and/or provincial government and/or public health authorities require it at any time during the season. Digital or printed paper QR code will be accepted. Proof of vaccination may be required to be with the participant at all times.

Registered students 12 years of age and older, parent participants, adult students 18 years of age and older, volunteers, assistants & instructors with medical exemption from for the covid-19 vaccination may be required to provide proof by enhanced exemption certificate with the QR code to enter the studio & participate in studio classes/activities if the federal and/or provincial government and/or public health authorities require it at any time during the season. Digital or printed paper QR code will be accepted. Proof of medical exemption from vaccination may be required to be with the participant at all times.

Registered students 12 years of age may have a grace period of 12 weeks from their 12th birth date to become fully vaccinated and show proof of full vaccination by enhanced vaccine certificate with the QR code to participate in studio classes/activities if federal and/or provincial government and/or public health authorities require it at any time during the season. They may still participate in studio classes/activities during the grace period. Digital or printed paper QR code will be accepted.

Registered students under 12 years of age may be required to provide proof of full vaccination plus 14 days to be shown through enhanced vaccine certificate with the QR code to enter the studio & participate in studio classes/activities if the federal and/or provincial government and/or public health authorities require it at any time during the season. Digital or printed paper QR code will be accepted. Proof of vaccination may be required to be with the participant at all times.

The Verify Ontario App will be used to verify full vaccination/medical exemption certificates if the federal and/or provincial government and/or public health authorities require it at any time during the season. No reporting, copying or retaining of record will be done.

If changes are made in the position of the federal and/or provincial and/or public health authorities or in the number or severity of Covid-19 cases and/or health guidance and a vaccine mandate is imposed and you choose to discontinue your child’s and/or your participation at PDDA activities and/or classes a refund or credit will not be issued for fees paid.

Visit the sanitization station upon entrance & exit of the studios.

Arrive dressed in dance wear. Limit number of items brought into the studio.

Bring own FULL water bottle. Cups will not be provided for drinks.

I hereby consent to having the student(s) named on the registration form receive dance instruction from the Port Dover Dance Academy.

I agree the Port Dover Dance Academy or any of its teachers, assistants or volunteers will not be held responsible for any illness, bodily injury, lost or stolen goods.
